We work with all of our clients offer affordable care solutions based on a client’s insurance coverage and financial ability.
01. What services do you provide?
We provide both skilled nursing and aide services direct to our clients homes. Our skilled nursing services are typically for clients who have a medical condition (e.g., ventilator or tracheostomy tube) that requires ongoing support from a medical professional for 4 to 24 hours per day. Our home health aide services are typically for clients who need support with non-medical activities of daily living (e.g., bathing and toileting) that requires ongoing support from a certified aide for 6 to 24 hours per day. Additionally we provide both skilled nursing and aide respite care providing temporary relief for a primary caregiver, enabling them to take a much-needed break from the demands of caregiving a sick, aging, or disabled family member.
02. What geographic areas do you service?
We provide services in Los Angeles County, Ventura County, Orange County, San Bernardino County, and Riverside County.
03. Will insurance cover me or my loved one?
If you or your loved one has a condition that requires our services you are very likely covered by insurance or by a social services support organization. We take a wide range of insurance including Medi-Cal, Medicare and many private insurance carriers. Our team will work with you to find the funding to cover the services that you need.
04. Who are your medical professionals and caregivers?
Our provider staff are all licensed and trained medical professionals and home health aides. They are all screened thoroughly and provided with an in depth orientation and training to ensure they are of the highest quality. Depending on the level of care required by our clients we support them with Registered Nurses (RNs), Licensed Vocational Nurses (LVNs), Certified Nursing Assistants (CNAs), and Home Health Aides (HHAs).
05. What kind of support do you provide your clients?
We have a fantastic team that is dedicated to providing the highest level of service to our clients and addressing any issues or concerns that may arise. Each client is assigned a client care coordinator that is their direct contact throughout their time with Prime Home Health. We have a robust client care team where someone will always be available 24/7.
